Provider Access Information (CHIP)
What is CHIP?
CHIP is the Claims and
Health Information
Portal, an online tool
provided by the City of Mesa Employee Benefits Office for providers
to view member eligibility information any time.
Provider Access Information
To obtain provider access to CHIP, please:
- Print Provider
Interest Letter/Form, download
Adobe Reader for free, if
needed.
- An authorized person completes the form such as a doctor,
dentist, office manager, etc.
- Mail or fax back form to the City of Mesa at the address/fax number
listed on the form.
Once we receive the form, the authorized person will receive information
on accessing CHIP. Please
allow up to 5 business days to receive the information.
Claims Submission Information
f you are a Blue Cross Blue Shield of Arizona (BCBSAZ) provider, you can
submit your claims electronically through BCBSAZ. Refer any questions
about electronic claims submission to your
BCBSAZ Representative. Please do
not fax claims!
Our paper claims mailing address has changed. Please
submit all papers claims to:

If you are using a clearinghouse, please
provide them with this address as well.
Checking Claims Status
Processing time varies, but typically medical and dental claims process
within 14 days of receipt.
Providers using CHIP will need a valid ID and password to
check claim status. Status may also be obtained via phone with
Employee Benefits at (480) 644-2299 option 3.
