Special Enrollment Notice
An employee who elects no City plan coverage because of coverage under
another plan, may be permitted to enroll himself and dependents in the City plan if eligibility under that other coverage ceases,
or if the other employer stops contributing toward that
coverage. If this occurs, enrollment in the City plan must be requested within 31 days
of the other coverage ending (or within 31 days after the employer stops
contributing toward the other coverage).
Also, if a new dependent needs to be added as a result of marriage, birth,
adoption, or placement for adoption, the employee may be permitted to enroll in City
plans.The request to enroll must be made within 31 days after the marriage, birth, adoption, or
placement for adoption.
