Together in Service is a new initiative to inspire collaboration, innovation and creativity between the city and the faith community to meet the needs of Mesa.

Please complete the form to register as part of Together in Service. Your registration will inform us of opportunities to strengthen collaborations with the Faith-Based Community and maximize resources in addressing social service needs in Mesa.

Note: We consider the Faith-Based Community to include an organization or place of worship with a mission that is grounded in faith beliefs or practices, or a faith-based charitable organization or nonprofit.

This form will take 5-10 minutes to complete. All are welcome and appreciated for participating.
