Records & Reports

MFMD works directly with the public, legal representatives, or other agencies to provide information, answer inquiries, and respond to sensitive requests.

Depending on the format of requested records, there may be an associated fee. See page 3 of Fees & Charges(PDF, 2MB).

Fire Reports

Basic Fire Report Request - Includes date, time, location, dispatched resources, and narrative (frequently used for insurance claim documentation). Basic Fire Report requests must be made through the records request portal. No email, phone, or fax requests will be accepted.

MRDC Recording Request - Internal or Interagency requests for 911 recordings.

Medical Reports

Medical records are protected by HIPAA rules and regulations. These reports will only be released to and/or with:

  • Patients
  • Attorneys with a Medical Authorization Release form
  • Power of Attorney documents
  • Parents of minors: must have a copy of birth/death certificate as well as a notarized signature
  • Spouses or next of kin without power of attorney: must have a copy of death certificate as well as a notarized signature
  • Police agencies
  • Subpoenas

In the event of a death: The next of kin may obtain medical records with a copy of either the death certificate or power of attorney documents, along with a letter of request. 

Attorney requests:  A request on letterhead indicating the patient's name, date, time and location of incident along with a Medical Authorization Release Form signed by the patient/power of attorney and a self-addressed stamped envelope.

The Notice of Privacy Practices is relevant to patients treated or transported by Mesa Fire and Medical Department. The Notice describes how medical information about you may be used and disclosed and how you can get access to this information.

Property Records

Commercial-purpose property records searches include inspection records and hazardous materials incident reports for a specified address.

  • Results include the previous three years of inspection reports and hazardous materials history.

  • Record searches are limited to one address per request. Be sure to include the full address, as data is stored by address, not parcel number. 

  • All requests must be made through the Public Records Portal.
