The Mesa Fire and Medical Department and Mesa Public Schools will be offering two vaccination sites for adults and children ages 12 and up to receive the COVID-19 vaccine.

Pfizer
• First and second dose available to recipients 12 years and older. Parent or guardian must accompany the child to the vaccination appointment.
• Third dose is available 28 days after second dose to *immunocompromised individuals ONLY.
• Valid form of identification for adults (license, passport, etc.)
Moderna
• First and second dose available to recipients 18 years and older.
• Third dose is available 28 days after second dose to *immunocompromised individuals ONLY.
• Valid form of identification (license, passport, etc.)
Janssen
• Must be 18 years and older.
• Valid form of identification (license, passport, etc.)
• Second dose has not been authorized by CDC.
*Immunocompromised qualifications per CDC: active cancer treatment for tumors or cancers of the blood, received an organ transplant and taking medication to suppress immune system, moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome), advanced or untreated HIV infection, and active treatment with high-dose corticosteroids or other drugs that may suppress your immune response per CDC.
