Pfizer vaccine

• First and second dose available to recipients 5-17 years. Parent or guardian must accompany the child to the vaccination appointment.
• Booster is available 6 months after completing Moderna or Pfizer series for:
  • 65 and older
  • 18+ with *underlying medical conditions
  • 18+ who work in high-risk settings
  • 18+ who live in high-risk settings
• Valid form of identification for adults (license, passport, etc.)

*Underlying medical conditions per CDC: cancer, chronic kidney disease, chronic lung diseases (asthma, COPD, etc.), Dementia or neurological disorders, Diabetes I/II, heart conditions (heart failure, hypertension, etc.), HIV infection, liver disease, overweight/obesity, pregnancy, Sickle Cell Disease/Thalassemia, smoking (current/former), organ/blood stem cell transplant, stroke, substance use disorders, and immunocompromised state (weakened immune system).
