Childhood Immunization Clinic

When: Second Wednesday of the Month
Time: 5:00 pm -7:00 pm
Where: Superstition Springs Center 6555 E Southern Avenue, Mesa

Eligibility requirements:
•Birth to 18 years of age
•Child is uninsured
•Child is insured through AHCCCS
•Child is covered by an insurance plan that does not cover immunization costs
•Child is a Native American or Alaskan Native

*Please ensure to bring your child's immunization records to the clinic.
Children with private insurance that covers 100% of immunizations costs must visit their provider.
