Before getting started on your new online Mesa General Business License Application, please make sure to have the following information ready:
- Computer or smart phone with internet access, including
- Microsoft Edge or Google Chrome browsers
- Scanning capability
- $10.00 nonrefundable application fee, payable via credit/debit card or banking account
- Note: 2.37% service fee charged added to all credit/debit card transactions

- Basic information about your business including:
- Name, contact phone number, and email address of all individuals with 10% or greater interest in the business or corporation officers.
- Federal Employer Tax ID Number (FEIN)
- North American Industry Classification System (NAICS)
- This is the standard used by Federal agencies to classify businesses.
- Find your NAICS
- For corporations, LLC, and partnerships
- Articles of Incorporation, Articles of Organization, Partnership Agreement (whichever is applicable)
- Articles of Incorporation, Articles of Organization, Partnership Agreement (whichever is applicable)
- For sole proprietors and joint ventures
- For businesses that engage in taxable activities
- Valid Arizona Transaction Privilege Tax License (TPT) with Mesa business location added
- If your business does not have an existing TPT license, apply online through the Arizona Department of Revenue
- For all:
- Government-issued photo identification card for at least one individual with 10% or greater ownership interest or shares in the business, corporate officer, or business local primary contact.
